Video Submission Page

The final step to your feedback is to upload your tennis video. There are 2 ways to do this:

1. Upload video and fill out the required information via Google Forms. File size must not exceed 1 GB. The link is https://docs.google.com/forms/d/e/1FAIpQLSf6-2sOGrLp43Oelqun1J9c_lbTgPEHbnJi4IS-LsDnscsrQw/viewform?usp=dialog

OR

        2. Upload video via Youtube(unlisted, if you do not want it seen) or Vimeo(unlisted) and send us the link through the form below.

NOTE: Only ONE form is necessary, fill out either the Google Forms or the form below.

Filling out the form: The required materials for the form are the order number(which you received through your designated email) and your tennis video(link or upload via Google Forms). If you do not yet have an order number, please visit the Plans page and choose a plan before proceeding.
